Subtype Change and Treatment Refractoriness at the Time of Recurrence in a Patient with Breast Cancer (PubMed, Gan To Kagaku Ryoho)
At the time of tracheotomy, biopsy evaluation revealed that the metastatic left supraclavicular lymph nodes were ER- and HER2-positive; therefore, the treatment was switched to a trastuzumab, pertuzumab, and docetaxel(TPD)combination for HER2-positive recurrent breast cancer. However, PET-CT revealed increased accumulation in the recurrent lesions, and the treatment was switched to trastuzumab deruxtecan (T-DXd). The accumulation of recurrent foci became less pronounced and the patient continued to progress without further deterioration.

Racial disparity in pro-metastatic tumor microenvironment in treatment naïve breast cancer. (PubMed, NPJ Breast Cancer)
However, there was a statistically significant interaction between race and high TMEM doorway score with respect to distant recurrence risk in ER + /HER2- patients; Black patients with high TMEM doorway score were 4.6-fold (95% CI 1.28-22.82, p = 0.03) and 4.2-fold (95% CI 1.17 - 18.23, p = 0.04) more likely to have a distant recurrence at 5-years and 10-years, respectively, while White patients with high TMEM doorway scores did not (p = 0.21, p = 0.11). Our study reveals racial disparities in the TME of women with ER + /HER2- breast cancer, which may play a critical role in driving disparities in breast cancer outcomes.

Guestimating Molecular Subtyping of Breast Cancer by Ki67 in the Era of Artificial Intelligence. (PubMed, Int J Breast Cancer)
Our data provides a snapshot of the real-world allocation of multigene testing in early breast cancer, and supports other studies in highlighting the discrepancy between IHC-based and gene-based luminal subtyping. Ki67 evaluation remained consistent over time, and the use of AI for Ki67 scoring did not enhance the accuracy of IHC-based luminal subtyping.

Alpelisib-induced severe interstitial lung disease: case report. (PubMed, J Chemother)
We report a 79-year-old non-smoking woman with ER-positive/HER2-negative breast cancer who initially underwent breast-conserving surgery and adjuvant anastrozole in December 2023. Following local recurrence in October 2024, she received mastectomy and ribociclib-fulvestrant...Alpelisib was discontinued, and high-dose corticosteroids led to complete clinical and radiological resolution. This case highlights early-onset alpelisib-induced pneumonitis at a reduced dose and underscores the importance of early recognition and prompt management.
