Characterization of DICER1 Mutations in Clinically Aggressive Thyroid Carcinomas from a Large Database of Real-World Patient Samples (USCAP 2023)
DICER1 MT thyroid cancers make up a fraction of advanced stage, clinically aggressive thyroid cancers. Here, we found that they are most common in middle age to older adults even though prior studies have shown that DICER1 mutations account for most adolescent PDTCs and are present in a much larger percentage of pediatric thyroid nodules overall compared to adult nodules. We speculate that a small subset of indolent pediatric DICER1 MT thyroid tumors develop additional mutations leading to more aggressive behavior, and that this tumor progression occurs later in life.

FISH for MYC amplification and IHC for MYC overexpression are useful in distinguishing PRAS from AVLs and may also have prognostic value in secondary AS. In contrast, these methods have little diagnostic or prognostic value in primary AS and should not be used to distinguish primary AS from benign vascular neoplasms.
