    Some fun facts about CMML: TET2/SRSF2 mutations are initiating events but acquisition of ASXL-1, RUNX1, SF3B1 and DNMT3A mutations usually give rise to dysplastic CMML while ASXL1, JAK2 and RAS give rise to proliferative CMML. TP53 mutations are extremely rare in CMML ( <1%). (@Persian1010)

    Benefit of Vyxeos in secondary AML comes almost entirely from subgroup w DNTM3A and TET2 mutations. Didn’t help at all for TP53mut. Ven/aza inferior in TP53 Mut vs wt although good CR rate in poor risk cytogenetics w p53wt. Data from @ColemanLindsley & @DanPollyea #ASH22 (@Jmarksloan)

    The answer was angioimmunoblastic T-cell which often expresses CD10. Lymphoma of T-follicular helper cells. Often with mutations in TET2, DNMT3A, RHOA. Difficult to treat, optimal induction unknown CHOP CHOEP BC-CHP (if CD30+>10%) Consolidate with auto? Who knows. (@AaronGoodman33)

    Hematology Board Review A 50 year old with no medical history is diagnosed with AML. Cytogenetics reveal a normal karyotype. NGS reveals no mutations other than a TET2. 7+3 with daunorubicin and cytarabine is started. What does of daunorubicin do you recommend? @Berninini (@AaronGoodman33)

    AML Question! 70 year old patient with no major medical co-morbidities presents with pancytopenia. Bone marrow biopsy reveals AML with a normal karyotype. Mutation analysis reveals NPM1+ and negative for FLT3 ITD/TKD. A TET2 mutation was also identified. You recommend... (@AaronGoodman33)

    TET2 mutation was associated with longer OS in lung cancer pts treated with check point inhibitors. (Zhao, Salgia, Lung Cancer 2020). No TET2 inhibitor available yet. TET1 inhibitor is available but TET1 has opposite function. So Avoid Vit C during immunotherapy or add It? (@DanZhaoMD)

    So, what did we find? Pretty cool. A patient with two DNMT3A mutations, a TET2 mutation, inversion 16, trisomy 8? Five totally independent clones (1 wildtype, 3 CHIP, 1 AML). DNMT3A and TET2 mutations were not related to his leukemia (@DrChrisHourigan)
